We have two convenient locations to serve you better. Our Bountiful location is located at 506 S. Main Street #200, Bountiful, UT 84010, and can be reached at (801) 936-8000. Our Layton location is located at 1992 W. Antelope Drive, Suite 1F, Layton, UT 84041, and can be reached at (801) 820-5482. Both locations are open Monday through Friday from 8:30 am to 6:00 pm and are closed on Saturday and Sunday.
